IDD Direct Support Professional- PASRR

The IDD PASRR Direct Support Professional will work with individuals, families, and nursing facilities to provide support for IDD PASRR Positive individuals. Frequent travel to nursing facilities and community settings in Collin County is required for this position. Other duties may be assigned to meet the needs of the program and individuals served.  Work is performed under the general supervision of the PASRR Supervisor with broad latitude for the use of initiative and independent judgment. 

The IDD PASRR Direct Support Professional exemplifies service excellence by providing direct care support to those individuals in the PASRR Independent Living Skills Program by supporting goals and outcomes identified in their service plan. The IDD PASRR Direct Support Professional is responsible to maintain scheduling for each client to provide Independent Living Skills to include outings in the community, attending on site events with the individual at the nursing facility, and or transporting to day habilitation/behavior support, according to their personalized plan and person-centered preferences, and services will be conducted with a trauma-informed and person-centered approach. IDD PASRR Direct Support Professional is responsible to maintaining a valid/current Texas Driver's License enabling you to legally drive at all times. Engagement with individuals served and exceptional customer service will be provided and documented in the electronic health record at a minimum 50% of work time for staff.

The IDD PASRR Direct Support Professional practices good stewardship of resources by ensuring all required trainings are completed prior to due date and center-issued equipment is kept in good repair. Knowledge and implementation of all Company policies/guidelines per the policy and Employee Handbook/Guidebooks is expected.

Integrity is necessary to ensure adherence to the Health and Human Services Commission, Texas Administrative Code, contracts, grants, and Lifepath Systems policies and procedures. Documentation which meets fidelity standards and documented into the electronic health record within 2 business days at a rate of 90% or above is expected. 

The IDD PASRR Direct Support Professional builds positive relationships with community-facing agencies and always maintains a professional relationship with all community stakeholders and individuals in services. Professional conduct with peers, leadership, and support teams is expected at all times and maintaining close correspondence with the IDD PASRR Habilitation Coordinator and PASRR Supervisor to report an individual’s progress or lack of progress towards outcomes and goals is essential. Promoting choice and autonomy through a strength-based model is necessary to assist individuals served with accessing needed supports.

If Bilingual and receiving stipend, employee will be available to translate for other staff as needed and will utilize approved language in the provision of services as needed.

Education, Training and Experience (including licensure and certification)

• High School Diploma or GED.
• At least 6 months of experience working with People with Development Disabilities.
• Minimum age of 18 years of age.
• Must be a licensed driver in the state of Texas.
• Must have reliable transportation and a valid driver’s license.

Knowledge, Skills, and Abilities

• Knowledge of human services delivery system.

• A working knowledge of the PASRR program and ILS rules as well as LifePath Systems rules, policies, and procedures.
• The ability to communicate, orally and in writing in English.
• The ability to organize and prioritize a variety of assignments while managing time effectively and efficiently.
• The ability to form and maintain effective relationships with the individuals served, families, stakeholders, co-workers, and the public.
• The ability to link plans and support activities to the outcomes identified by the individuals served.
• The ability to document and demonstrate support activities and actions based on the outcomes identified by the individuals served.
• Ability to solve practical problems and deal with a variety of concrete variables.
• Ability to interpret instructions provided in written, oral, diagram or schedule form
