Position Overview
We are seeking a dedicated and compassionate Bachelor’s Level Outpatient Case Manager to provide wraparound, community-based services to individuals and families in the post-crisis phase or following a behavioral health challenge event. This role focuses on stabilization, connection to resources, and ongoing support to promote long-term success in the home and community.
The ideal candidate is highly motivated, organized, and comfortable working in community-based settings. This position includes in-person, community-based visits, along with close collaboration with internal team members and external partners to ensure coordinated, high-quality care.
Key Responsibilities
Wraparound & Case Management Services
- Provide comprehensive, family-centered case management during the post-crisis phase or following a behavioral health challenge event
- Support stabilization and reduce risk of escalation or need for higher levels of care
- Develop, implement, and monitor individualized service plans based on client and family needs
Outpatient & Community-Based Support
- Conduct visits in home and community settings to assess needs and provide ongoing support
- Assist clients and families in accessing, coordinating, and navigating community resources and services
- Reinforce progress and support continued stabilization in the post-crisis phase or following a behavioral health challenge event
Collaboration & Care Coordination
- Work as part of a multidisciplinary team to support client and family goals
- Coordinate care with community partners, including schools, social services, medical providers, and other professionals
- Support families in navigating systems of care during the post-crisis phase or following a behavioral health challenge event
Documentation & Compliance
- Maintain timely, accurate, and compliant documentation, including service plans and progress notes
- Complete required reports for internal and external stakeholders
Professional Development
- Stay informed on best practices in community-based and wraparound services
- Participate in supervision, training, and ongoing professional development opportunities
Travel
- Travel locally to provide services in home and community-based settings
- Utilize a personal vehicle for work-related travel (mileage reimbursement provided)
Additional Duties
- Perform other duties as assigned to support program needs and client care
Qualifications
- Bachelor’s degree in Social Work, Psychology, Counseling, or a related field
- Experience in case management, family support, or related roles preferred
- Experience working with individuals and families in the post-crisis phase or following a behavioral health challenge event preferred
- Ability and willingness to travel locally
- Strong interpersonal, organizational, and communication skills
- Ability to work independently and as part of a collaborative team
- Knowledge of community resources and social service systems
- Commitment to cultural humility and working with diverse populations
- Valid driver’s license and reliable transportation

Pay: $21.00 - $24.00 per hour
