What are the responsibilities and job description for the Advanced Practice Registered Nurse - Primary Care position at Collaborative Medicine LLC?
Collaborative Medicine LLC is a primary care and medication management practice owned and operated entirely by Advanced Practice Registered Nurses (APRNs). We take pride in offering expert, patient-centered care with a compassionate and down-to-earth approach.
We are not a corporate environment—we believe in forming genuine, lasting relationships with our patients. We take insurance because we believe care should be accessible to everyone, with one goal in mind: to restore trust between patient and provider by allowing clinicians the time they need to truly listen, diagnose, and treat effectively.
Our Philosophy
In a healthcare landscape dominated by overbooked schedules and rushed visits, we stand apart. We do not double-book patients. We encourage independent APRNs to practice with integrity and autonomy—delivering thoughtful, quality care in a supportive environment that values your professional time and clinical judgment.
Our goal is also to improve the lives of independent APRNs by providing the infrastructure, administrative support, and flexibility needed to thrive in private practice—without the corporate red tape.
Position Overview
We’re seeking a Licensed APRN (Primary Care) to join our team on an independent contractor (1099) basis. The ideal candidate is a compassionate, detail-oriented clinician who values time with patients and wants to work in an environment where quality care comes before quotas.
As an independent provider, you will set your own patient volume and schedule—whether that’s five days a week or three days a week. You’ll have the flexibility to determine your own goals, patient load, and charting pace.

- Electronic Health Record (EHR) system

- Medical assistant, nurse, and receptionist support
- Fully equipped exam rooms and medical supplies

- Maintain accurate and timely chart documentation in the EHR.
- Collaborate with clinical and administrative staff to ensure smooth patient flow.
- Adhere to state and federal regulations governing APRN practice and prescribing.
- Participate in quality improvement initiatives and team communication as needed.
Qualifications
- Current Connecticut APRN license in good standing with ability to practice independently without MD supervision
- Active national board certification (ex. AANP)
- DEA and CT controlled substance registration (or eligibility to obtain)
- Your own active malpractice insurance (or willingness to obtain prior to credentialing).
- Commitment to compassionate, patient-centered care
- Desired: Minimum 1 year of independent primary care experience
Ideal Candidate
- Enjoys working independently but values being part of a supportive team
