Coleman Health Services has been selected to be part of OhioRISE to work with Multisystem Involved Youth and their families! If you want to make a difference, we want you on our team! As a CME Intake Specialist, you will complete CANS assessments and be the primary point of contact for all youth referred to OhioRISE in Richland County. The position will be based out of our Mansfield, Ohio, office.

What are we looking for from you?

Ability to conduct comprehensive assessments including the CANS assessment

  • Assist in the enrollment, triaging, and immediate care needs of youth hospitalized due to a behavioral health condition.
  • Actively communicate with referents to obtain collateral information and provide referral status updates.
  • Ensures a warm handoff to the assigned care coordinator following admission to OhioRISE.

What Qualifications will you need to be successful as a CME Intake Specialist (Youth/Young Adults)?

  • High school diploma or equivalent with 3 years' experience (or 2 years' experience with an associate's/bachelor's degree or 1 year of experience with a master's degree) in children's behavioral health, mental health, or a related public sector providing community-based services to children and youth and their families or caregivers
  • Ability to manage complex cases to work with children and families who are engaged in one or more child-serving systems
  • Able to effectively communicate with youth, caregivers, multi-disciplinary staff, and partner agencies
  • Have and maintain an excellent driving record
  • Able to pass BCI and FBI background checks

Preference given to the following candidates:

  • Master's degree with LSW/LISW/LISWS, LPC/LPCC/LPCCS, MFT/IMFT/IMFTS
  • Has NPI and Ohio Medicaid numbers
