Seeking a full time Case Manager to provide case management services to individuals with severe and persistent mental health issues who may also have a substance abuse disorder in Mahoning County.

Job Duties of Case Manager includes, but not limited, to:
- Assist with accessing community based resources, housing, employment and linking the individual with ongoing mental health and/or substance abuse services.
- Transition clients and families into ongoing care
- Work collaboratively with treatment teams internally and externally to ensure safe transitions
Qualifications of the Case Manager
- Bachelor's degree in a social service-related field or three-years' experience
- Willing to get NPI and Medicaid Billing number (we'll help!)
- Maintain excellent driving record
