Coleman Health Services is seeking a Case Manager (Youth) for Portage County, Ohio. A Case Manager (Youth) provides care coordination and other case management services to youth and adolescents in Portage County who have mental health and substance use disorders. This position is based in Kent, Ohio.

Duties of Case Manager (Youth) include:
- Coordinate ongoing care with internal and external healthcare providers
- Assist with symptom monitoring and coping skill development
- Assist with making healthcare, service, and community resource referrals
Minimum Qualifications of Case Manager (Youth) include:
- BA/BS in mental health/related field or three years' related experience
- Have/obtain a NPI number and an Ohio Medicaid number
- Have/maintain an excellent driving record
Preferred Qualifications for Case Manager (Youth):
- BA/BS in mental health/related field
- Experience in case management
- Experience working with adults in a community mental health setting
