Key Responsibilities:
- Maintain a caseload of 8 children in the adolescent IDT program, 6th – 12th grade.
- Participate in IDT admissions with child, family, MD, and school liaison.
- Provide Weekly Individual Therapy.
- Provide Daily Group Therapy.
- Provide Family Therapy and Crisis Intervention as needed.
- Facilitate Transition Meetings for each student on their caseload that include the student and family, IDT teacher, school liaison and other professionals from the student’s home district.
- Collateral involvement with participating schools and agencies.
- Function as part of an interdisciplinary treatment team.
- Participate in weekly Treatment Team meetings.
- Advocate for youth and families to obtain community-based linkages with additional supportive services that meet their needs.
- Maintain timely medical record documentation.
- Facilitate thorough discharge planning and complete discharge follow-up.
- Collaborate with school liaisons throughout Orange County to facilitate the referral and screening process.
