SUMMARY DESCRIPTION:

The Outreach Care Coordinator is a field-based position within the Aberdeen Cares Team (ACT), a partnership between the Coastal Community Action Program (CCAP) Housing & Community Services (HCS) department and the City of Aberdeen. Outreach Care Coordinators work split AM or PM shifts covering early morning through late evening hours, and serve as the primary point of contact for individuals experiencing homelessness in Aberdeen’s downtown corridor. The role involves accompanying City staff on downtown wellness checks and wake-up outreach ahead of business hours, following up on contacts made during the prior shift, and connecting individuals to CCAP services and external treatment and homeless housing providers.

This position is supported through a combination of City of Aberdeen funding and Foundational Community Supports (FCS) Housing billing, and is designed to be sustained through consistent billable activity. Outreach Care Coordinators play a critical role in engaging individuals who are otherwise disconnected from services and linking them to housing stability and treatment resources. During the initial launch phase, Outreach Care Coordinators accompany the City’s ACT Coordination Lead on morning field rounds as part of onboarding to Aberdeen’s downtown corridor, frequent utilizers, and the City’s existing wellness-check practices.

FUNCTIONAL RESPONSIBILITIES:

Field Outreach & Participant Engagement

● Arrive at shift start and review field activity and shift reports from the preceding shift to identify individuals to follow up with.

● Accompany City staff on downtown wellness checks and wake-up outreach to engage individuals sleeping downtown ahead of business hours (AM shift).

● Engage individuals during outreach using trauma-informed, strengths-based approaches to build trust, support goal progress, and identify barriers to housing stability.

● Connect individuals to CCAP services and external treatment, detox, and homeless housing providers based on identified needs.

● Model appropriate boundaries, self-advocacy, and accountability in all participant interactions.

● Relay relevant updates, concerns, or barriers to the ACT Program Coordinator and backup Care Coordinators in a timely manner.

Shift Handoff & Coordination

● Document all contacts made during the shift in a shift report, capturing who was contacted, location, need identified, and any commitment made.

● Review the incoming shift report at shift start and follow through on commitments made to individuals contacted during the prior shift.

● Coordinate with backup Care Coordinators to ensure continuity of coverage during the daily AM/PM handoff gap and scheduled days off.

● Maintain proactive communication with City partners, Fire/EMS, and Aberdeen Police Department regarding shared referrals and field activity.

● Adapt daily schedule as needed to respond to field conditions, including weather, referrals, and time-sensitive wellness checks.

FCS Billing, Documentation & Program Expectations

● Accurately document all FCS Housing billable activities in required agency systems on the date of service or no later than the following business day.

● Maintain detailed service logs including participant name, date, service type, duration, and activity summary sufficient to support billing and program review.

● Meet a program expectation of a minimum of 20 billable FCS Housing units per month. This expectation reflects the level of activity required to sustain the position and will be monitored by the ACT Program Coordinator as a program viability metric.

● Understand and apply FCS Housing billing standards, including the one-unit-per-client-per-day rule, and ensure all documentation supports compliant billing.

● Participate in regular supervision with the ACT Program Coordinator and provide updates on billable activity, caseload engagement, and any barriers to meeting program expectations.

● Submit all documentation, logs, and reports accurately and on time in accordance with agency and funder requirements.

Collaboration & Professional Representation

● Collaborate regularly with backup Care Coordinators, the ACT Program Coordinator, City of Aberdeen staff, Fire/EMS, and Aberdeen Police Department to coordinate participant support.

● Represent CCAP professionally when interacting with City staff, law enforcement, Fire/EMS, detox programs, Quinault Wellness Center, and other community partners.

● Participate in team meetings, trainings, and supervisory sessions as required, including field training with the City’s ACT Coordination Lead during the launch phase.

● Contribute positively to a collaborative, trauma-informed team culture across the ACT program.

MINIMUM QUALIFICATIONS:

● Experience working with or supporting individuals experiencing homelessness, behavioral health conditions, or substance use disorders.

● Ability to work a fixed split shift, including early morning (as early as 5:00 a.m.) or late evening (until midnight) hours, and weekend days as assigned.

● Current Washington State Driver’s License and safe, clear driving record.

● Strong interpersonal and communication skills with the ability to build trust with individuals experiencing crisis or instability.

● Ability to work outdoors in varied weather conditions and navigate a downtown field environment.

● Ability to maintain confidentiality and appropriate professional boundaries.

● Ability to pass and maintain a comprehensive background check.

● Proficiency in basic documentation and time management sufficient to meet shift-reporting and billing requirements.

PREFERRED QUALIFICATIONS:

● Certified Peer Counselor (CPC) certification, or active progress toward obtaining it.

● Lived experience with homelessness, substance use recovery, or housing instability.

● Familiarity with Foundational Community Supports (FCS) Housing program requirements and billing practices.

● Knowledge of Aberdeen and Grays Harbor County community resources, service providers, and the downtown corridor.

● Experience with electronic documentation systems, case management software, or HMIS.

● Bilingual – English/Spanish.

● Motivational interviewing skills or training in trauma-informed care.

● Prior experience working alongside law enforcement or municipal field staff on outreach or wellness checks.

LOCATION / WORKING HOURS:

This is a full-time, hourly position based in downtown Aberdeen, WA. Outreach Care Coordinators are assigned to one of two fixed split shifts: morning shift, 5:00 a.m. – 2:00 p.m., Sunday through Thursday; or evening shift, 2:00 p.m. – 11:00 p.m., Tuesday through Saturday. Each shift includes a one-hour unpaid lunch. This position works primarily within the downtown Aberdeen corridor; occasional local travel may be required to accompany participants to nearby services or appointments. These hours are subject to change as the program evolves.

This position is subject to pre-employment drug testing.
