We are seeking a compassionate and knowledgeable Registered Dietitian (RD/RDN) to join our Primary Care team. The ideal candidate will provide individualized medical nutrition therapy to patients with a variety of conditions, including obesity, diabetes, cardiovascular disease, gastrointestinal disorders and other chronic or acute conditions. You’ll work closely with physicians, nurses, and other healthcare professionals to support optimal patient outcomes through nutrition-focused interventions and education. 

Registered Dietitian (RD/RDN) Responsibilities:

• Conduct comprehensive nutrition assessments and develops individualized care plans for patients and/or their families using evidence-based medical nutrition therapy 


• Complete nutrition assessment, determination of the nutrition diagnosis, developing and implementing evidence-based nutrition intervention(s), and monitoring and evaluating the patient's progress, and communicating the nutrition care plan with other members of the care team by effective verbal and written communication  


• Chronic disease management and prevention in conditions such as pre-diabetes, diabetes, obesity, kidney disease, etc.  


• Provides one-on-one and group nutrition counseling sessions for patients. 


• Provides appropriate follow-up in accordance with the patient's treatment goals; transition of care, referral, to other clinical professionals when appropriate 


• Collaborate with the interdisciplinary care team to coordinate patient care and ensure continuity of treatment. 


• Document all patient encounters in accordance with regulatory and organizational standards with accuracy and timeliness 


• Understand and comply with third-party payer's policies and regulations for ambulatory medical nutrition therapy services. 


• Stay current with research and best practices in clinical and outpatient nutrition and dietetic topics 


• Participate in quality improvement, community outreach or educational initiatives as needed 


• Maintain all professional credentials in good standing at all times during employment 

Registered Dietitian (RD/RDN) Education/Experience:

• Bachelor's degree in Nutrition or related field and a minimum of 3 years of clinical and nutrition related experience


• Excellent patient engagement skills and the ability to work in a fast paced, team environment 

Registered Dietitian (RD/RDN) Certificates and Licenses:

• Registered Dietitian Nutritionist (RD/RDN) in good and current standing with the Academy of Nutrition & Dietetics (AND), and the Commission on Dietetic Registration (CDR)


• Current New York State Certified Dietitian Nutrionist (CDN) 


• Desire to grow and become a Certified Diabetes Care and Education Specialist (CDCES) preferred.
