Position Description for Nurse Practitioner:
We are looking for a dedicated and compassionate
Nurse Practitioner for our Nursing Homes in Altamont, NY. This role is focused on delivering comprehensive, patient-centered dermatological care at the bedside, ensuring residents receive timely, effective, and high-quality treatment within a collaborative clinical environment. This is a full-time, in-person position Monday - Friday 9am - 5pm.
Qualifications for Nurse Practitioner:
- Current, unrestricted NP license in the state of New York
- Proven experience as a Nurse Practitioner in a skilled nursing or long-term care setting
- Expertise in primary care, acute care, and chronic care practices
- Knowledge of health and patient care regulations
Responsibilities for Nurse Practitioner:
- Provide comprehensive medical care to residents in a skilled nursing or long-term care setting
- Collaborate with the interdisciplinary team including physicians, nurses, and therapists
- Manage acute and chronic conditions for a diverse resident population
- Perform rounds and respond to changes in patient condition
- Document patient encounters in accordance with facility and regulatory standards
