We are looking for a dedicated NP or PA for our skilled nursing facility in Rochester, NY. This is a full-time, in-person position with a weekday schedule, no nights, and no on-call. The provider manages a panel of long-term care and post-acute residents, partnering with the facility's medical director and interdisciplinary team to deliver primary and chronic care on site.
Qualifications of the NP/PA:
- Active New York State license as a Nurse Practitioner or Physician Assistant
- Current national board certification (NP) or NCCPA certification (PA)
- Experience in a skilled nursing, post-acute, long-term care, or geriatric setting preferred
- Comfortable managing a resident panel independently with medical director support
Responsibilities of the NP/PA:
- Provide on-site primary and chronic care to skilled nursing and long-term care residents
- Conduct admissions, follow-up visits, and change-in-condition assessments
- Document accurately and coordinate care with the medical director and facility team
- Support regulatory compliance and quality measures within the facility
