Nurse Practitioner or Physician Assistant for our Skilled Nursing Facility (SNF)
Compensation: $115,000 - $150,000 Annual Salary

We are looking for a Nurse Practitioner or Physician Assistant for our Skilled Nursing Facility in Vineland, NJ who prides themselves on a positive attitude and dedication to providing top-notch care. This is a full time, in-person position. The provider rounds on skilled nursing and long-term care residents, manages acute changes in condition, chronic disease and advanced care planning, and coordinates with the interdisciplinary team and collaborating physician.
Qualifications:
- Active New Jersey NP (FNP or AGNP) or PA license in good standing
- DEA licensure
- Experience in a skilled nursing or long-term care setting preferred
- Ability to round independently and manage a medically complex geriatric panel
Responsibilities:
- Provide primary and preventative care to residents in a skilled nursing facility
- Manage residents with an acute change in condition
- Advanced care planning and chronic care management
- Order, review and follow up on treatments and diagnostic testing
- Communicate with facility nursing staff and collaborate with the primary care physician
