Job Overview
We are seeking a dynamic and dedicated Weekend Warrior Licensed Practical Nurse (LPN) to join our healthcare team. Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems such as Epic or Cerner.
- Administer medications and injections, including IV infusions and catheter care, following strict protocols to ensure safety and compliance with HIPAA regulations.
- Assist with complex procedures such as wound suturing, tube feeding, and airway management while maintaining sterile technique.
- Support acute pain management and infectious disease care by monitoring patient responses and adjusting interventions accordingly.
- Collaborate with multidisciplinary teams to coordinate discharge planning, case management, and long-term care strategies for patients with dementia, Alzheimer's disease, or other chronic conditions.
- Conduct patient triage over the phone or in person, utilizing skills in emergency medicine, urgent care protocols, and Level I trauma center experience to prioritize urgent needs.
- Document all patient interactions accurately in EMR/EHR systems like Athenahealth or eClinicalWorks, ensuring compliance with clinical confidentiality policies.
Skills
- Proven experience working with diverse patient populations including geriatrics, pediatrics, neonatal ventilator care, and individuals with developmental disabilities or disabilities.
- Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), CPT coding, and documentation standards.
- Skilled in IV insertion, phlebotomy, specimen collection/processing, and sterile processing techniques.
- Familiarity with EMR/EHR systems such as Epic, Cerner, Athenahealth or similar platforms for efficient medical records management.
- Excellent caregiver communication skills to effectively educate patients and families about medication administration, health coaching, and discharge instructions.
- Ability to work independently during weekend shifts while maintaining high standards of clinical excellence in settings like skilled nursing facilities or assisted living communities.
- Knowledge of long-term care regulations, hospice care principles, infection control practices, and acute/critical care procedures including ventilator management.
