Clive Behavioral Health
Clive Behavioral Health opened in February 2021 with 100 inpatient beds for adult, child & adolescent patients experiencing a severe behavioral health crisis. Care at this location focuses on crisis management, stabilization and development of a plan to return to community living with, if needed, supportive outpatient services and community services and resources.
Qualifications
The Care Navigator is responsible for identifying patients hospitalized at CBH who would benefit from stepping down to the PHP and/or IOP level of care. He/she also oversees the outpatient admission process for direct admissions. He/she will meet with all eligible patients to discuss the benefits of Outpatient services as part of their continuum of care treatment. The Step-Down Navigator will be responsible for communicating eligibility to key members of senior leadership & associated departments and will be responsible for compiling and tracking data to assist in meeting expectations for step-downs. It is expected that the Care Navigator will identify implementation barriers and evaluate the process with facility leaders using the data. A thorough understanding of the Inpatient workflow process, as it pertains to case management and discharge planning, is preferred.

Qualifications
Education: Bachelor’s Degree in mental health/behavioral sciences field required, Master’s degree preferred.
Experience: Minimum one (1) year experience in psychiatric/mental health field.
