Licensed Practical Nurse

Our client, a well-established senior living community, is currently seeking 3 compassionate and dedicated Licensed Practical Nurses (LPNs) to join their healthcare team. These positions support residents in both Assisted Living and Memory Care settings.

We are currently hiring for the following shifts:

• Day Shift: 1 opening
• Evening Shift: 1 opening
• Overnight Shift: 1 opening

Key Responsibilities

• Administer medications and treatments according to physician orders.
• Monitor residents' vital signs, including blood pressure, pulse, and temperature.
• Provide nursing care and assist with daily resident needs as necessary.
• Monitor changes in residents' conditions and communicate updates to physicians, families, and the nursing team.
• Maintain accurate documentation of medications, treatments, and resident care.
• Process physician orders and coordinate medication and medical supply requests.
• Complete narcotic counts and provide detailed shift reports.
• Respond promptly to resident emergencies and requests for assistance.
• Assist with resident admissions, discharges, and care coordination.
• Collaborate with nursing staff and other healthcare professionals to ensure continuity of care.
• Promote a safe, comfortable, and respectful environment for all residents.

Qualifications

• Active New York State LPN license required.
• Current CPR certification required.
• Minimum of 2 years of nursing experience.
• Previous experience in geriatrics, assisted living, long-term care, or memory care preferred.
• Strong communication, documentation, and organizational skills.
• Ability to work independently while collaborating with a healthcare team.
