Clinical Magnet (Part of ICON Medical) is seeking an experienced Director of Case Management (RN) to lead a talented team at our partner’s rehabilitation hospital in the Tallahassee, FL area. • Lead a department of dedicated RN and Social Worker Case Managers.
• Drive quality care and discharge planning in a 70-bed rehabilitation hospital.

What You’ll Do:

• Oversee the daily operations and staff management of the Case Management department.
• Guide interdisciplinary care planning and discharge processes for optimal patient results.
• Serve as a patient and family advocate while building strong relationships with payers and healthcare partners.
  

What You’ll Bring:

• Active RN license in Florida and BSN or higher.
• At least 3 years of hospital-based Case Management (acute care, inpatient rehab, or LTACH).
• 2 years of leadership experience (supervisor level or above).
• Strong skills in communication, organization, and collaboration.
