As a LPN Central Admission, you will facilitate the timely and appropriate initiation of home health services for all referred patients and coordinate with Cleveland Clinic’s homecare agency and outside partner agencies to confirm timely initiation of homecare services.
A caregiver in this position works 9:00am – 5:30pm and has the opportunity to work a hybrid schedule.
A caregiver who excels in this role will:
Assess the appropriateness of the requested home care services and skilled needs identified under the guidance of a Registered Nurse.
Coordinate initial and final orders on internal and external referrals for patients needing home care nursing, therapy and social work services.
Incorporate clinical and financial knowledge with referral process and recommend appropriate alternative approaches when appropriate.
Communicate the home care needs with scheduling, managers, reimbursement, and other parties involved in the home care services process.
Serve as the CCHCS information specialist.
Understand other services and products available through CCHCS, including criteria for service and reimbursement for all the home care business lines.
Minimum qualifications for the ideal future caregiver include:
Graduate from an accredited practical nursing program.
Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN)
Two years of bedside LPN experience
Proven office technology experience including electronic medical record system and MS office products
Preferred qualifications for the ideal future caregiver include:
One year of clinical experience in homecare.
Knowledge of and skills in Microsoft applications.
Experience with Epic Hyperspace and/or Careport/ECIN.

Physical Requirements:
- Requires sitting, standing and walking for extended periods of time. Requires manual dexterity to perform work on a PC.
- Requires good visual acuity through normal or corrected vision and must be able to hear typical conversation levels through normal or corrected hearing.
- Must be able to carry / transport office equipment including a desktop, laptop and monitor to home / work environment.
