- Provide regional oversight and leadership for all MDS processes across assigned facilities within the West Tennessee region, ensuring accuracy, consistency, and full regulatory compliance.
- Coordinate and guide interdisciplinary teams at each facility in completing the Minimum Data Set (MDS) for all new admissions and ongoing assessments within mandated federal and state timeframes.
- Ensure thorough, compliant, and high-quality documentation across multiple sites, maintaining exceptional standards aligned with organizational expectations and regulatory requirements.
- Maintain and monitor an accurate, region-wide schedule of all MDS assessments, including correct reference dates throughout each resident’s stay.
- Oversee and support timely and precise data entry and electronic submission of MDS assessments from each facility.
- Serve as a clinical resource for facility teams, assisting in the formulation and revision of individualized care plans that accurately identify resident needs, establish realistic goals, and outline appropriate interventions.
- Review and evaluate care plans across facilities for completeness, accuracy, resident-centered focus, and regulatory compliance.
- Assess goal progression, ensure appropriate reassessment, and guide care plan adjustments based on outcomes and changes in condition.
- Provide strong clinical leadership, training, and direction using consistent guidelines and resource materials to elevate MDS and care planning practices region-wide.
- Current Registered Nurse (RN) license in the State of Tennessee.
- MDS Certification required.
- Previous MDS experience in Skilled Nursing Facilities, with the ability to support and guide multiple locations.
- Strong working knowledge of MDS 3.0, assessment protocols, and care plan development.
- Understanding of Medicaid and Medicare requirements, including reimbursement processes.
- Long-term care experience with proven ability to lead, educate, and influence interdisciplinary teams across a regional footprint.
