Position Overview
As a Case Manager with Clear Path for Veterans New England, you will work closely with Veterans and Service Members to develop individualized stabilization plans that address housing, employment, and wellness needs. This role serves Veterans and their families across Cape Cod and Suffolk County, Massachusetts (MA), with a primary focus on outreach and client engagement in the Cape Cod area.
This is a field-based position supporting Veterans across Cape Cod and Suffolk County. The role requires regular travel throughout assigned service areas based on client and program needs.
The position begins with an initial in-person training period (approximately two months) at Clear Path locations in Devens and Methuen, MA.
This role combines direct client engagement with a significant documentation component. Case Managers are responsible for maintaining a full caseload while completing timely, accurate, and compliance-driven documentation for each client served.
Key Responsibilities:
Housing Stabilization Services
- Provide case management services to support housing stability.
- Develop assessments and Housing Stability Plans tailored to client needs.
- Administer Temporary Financial Assistance and connect clients with VA, state, and local benefits.
- Offer referrals to community resources and follow-up services to ensure progress.
Client Support and Advocacy
- Conduct intake assessments to determine program eligibility and client needs.
- Provide ongoing support through planning, implementation, and evaluation of client goals.
- Advocate for clients and connect them to essential services that align with their action plans.
Community Engagement
- Conduct outreach within assigned communities to connect with Veterans and Service Members in need.
- Represent Clear Path at local engagements and events.
- Conduct regular outreach throughout Cape Cod and Suffolk County based on program needs.
- Build and maintain relationships with community partners, service providers, and Veteran support organizations.
Administrative Support
- Maintain accurate and organized client files, including timely updates in the tracking system.
- Prepare data and narrative reports to support program evaluations.
- Ensure compliance with program requirements and deadlines.
- Manage a high volume of case documentation, including intake assessments, housing plans, case notes, and program-required reporting.
- Complete documentation in a timely and accurate manner to meet SSVF and internal compliance standards.
- Balance a dynamic caseload with significant administrative responsibilities.
Requirements
- Associate or bachelor’s degree in social work, human services, or a related field preferred; equivalent experience will be considered.
- 1-3 years of experience in case management or a related direct-service role, preferably within Veteran services or the human services field.
- Strong organizational, communication, and problem-solving skills.
- Ability to build rapport and effectively engage with Veterans from diverse backgrounds.
- Ability to work independently and collaboratively with at-risk populations.
- Proficiency in data management systems, such as HMIS or similar
- Reliable transportation and valid driver’s license required.
- Ability to manage a documentation-heavy workload in a fast-paced, compliance-driven environment.
Preferred Qualifications
- Military or Veteran experience is not required but is strongly valued.
- Passion for serving Veterans and helping remove barriers to housing and stability.
- Knowledge of homelessness prevention, rapid re-housing, and transitional support services.
- Ability to remain flexible and adapt to changing environments.

Compensation:
- The salary range for this position is $55,000 - $58,000 annually. The final offer will be based on experience, qualifications, and alignment with program funding.
