The Home Care Manager assists with the day-to-day operations of the Home Care Divisions. They carry out responsibilities in the following functional areas: staff management and development, scheduling, quality assurance, department growth, etc.

Key Responsibilities
- Provides direct supervision of staff performance of acceptance of initiative and responsibility, communication skills, interpersonal skills, attendance, safety consciousness, and problem-solving.
- Oversees approval of staff time off requests.
- Lead staff meetings and communicate Department & Organization changes and updates.
- Work with Human Resources in the recruitment and hiring process of Home Care staff to meet client and staffing needs.
- Follow all documentation completeness and reporting of incidents, client concerns and significant events.
- Ensure home care policies and procedures reflect quality and safe delivery of services.
- Works with Human Resources and Director of Home Care in staff coaching and disciplinary action as indicated.
- Manage the admission, assessment, and discharge of clients in compliance with all Federal and State regulations.
- Assure authorization and compliance for payment of services.
- Enter new clients into various databases for electronic charting and visit verification.
- Document and resolve all requests from clients and employees concerning changes in direct care staff and/or client schedules.
- Maintain and update authorized hours and client/employee scheduling placements.
- As a member of the admission team, receive referrals for services and ensure timely and efficient transmission of referrals to appropriate personnel to increase client satisfaction and increase overall client base.
- Assist in screening and completion of referrals to ensure clients meet agency admission criteria and comply with state and federal regulations.
- Cross-sell company services to all referrals as appropriate to increase client base and company profits.
- Coaches and develops individual team members by providing constructive and timely performance evaluations.
- Client related discipline- by supervisor of client care type being provided
- Maintain accurate documentation of services according to policies and procedures.
- Develops and maintains training material for new staff in areas of Managed Care, County, State and Federal laws, regulations and organization policies and contractual obligations of funders
- Sells program to primary funders including, but not limited to, MCOs, Counties and Private Pay sources.
- Maintain and strengthen relationships with primary funders of services.
- Develop and implement clear and measurable short and long term goals, operating procedures, and business plans to achieve the agency’s growth goals.

Desired Skills and Qualifications:
- A Bachelor Degree, in a Human Services or Similar Field, is preferred.
- At least five years of industry related experience including two years in upper management.
- Working knowledge and understanding of Federal and State laws and regulations pertaining to Home Care, DVR, Medicaid, and other insurance and payer sources.
- Must possess a valid WI driver’s license, and ability to meet agency driving standards. Must provide proof of auto insurance.
- Must pass a periodic WI Caregiver Background Checks including Department of Justice Criminal Record Checks, and/or that of previous states of residence.
