SUMMARY

The Memory Care Supervisor is responsible for planning, organizing, and directing overall operations in the Memory Care Cottage in accordance with residents' needs, government regulations, and Civitas Senior Living policies and procedures. This role reports to the Wellness Director and maintains shared accountability for both resident engagement and clinical oversight within the Cottage.

Pay Status: HOURLY, Non-Exempt
Equipment Provided: Company-issued mobile phone for on-call communication. Tablet will also be included for our Engagement Platform.


DUTIES AND RESPONSIBILITIES

The Cottage Programming & Resident Engagement

  • Plan, coordinate, and execute meaningful, person-centered programming that supports the physical, cognitive, emotional, social, and spiritual well-being of Memory Care residents.
  • Develop, adapt, and implement activities that align with the unique abilities, interests, and needs of residents living with dementia.
  • Create and maintain monthly Memory Care calendars that meet company programming standards and reflect resident preferences and engagement goals.
  • Ensure daily programming is executed as scheduled and adjusted as needed to meet resident needs and participation levels.
  • Lead and facilitate group and individual engagement opportunities that promote purpose, connection, and quality of life.
  • Train and support team members on resident engagement techniques, dementia-specific programming, PAC, Dementia Live, and other company-approved programs.
  • Accurately document attendance, participation, and resident engagement outcomes in designated company systems.
  • Upload photos, stories, and engagement highlights in accordance with company standards and communication expectations.
  • Develop and maintain relationships with volunteers, community partners, and third-party providers to enhance programming opportunities.
  • Coordinate and schedule volunteers, specialty programs, and community partnerships to support ongoing resident engagement.
  • Plan and facilitate family engagement initiatives, including Family Night and Walking With You Support Groups, when applicable.
  • Collaborate with community leadership and care teams to integrate engagement into daily resident life and support individualized care plans.
  • Monitor resident participation and engagement trends and adjust programming strategies to maximize involvement and meaningful outcomes.

Cottage Environment

  • Ensure the safety of residents through appropriate security measures and ongoing staff training.
  • Maintain a Cottage environment that is free from odor, clean, clutter-free, and tour-ready at all times.
  • Ensure life skill stations, kitchenette, furniture, signature scent, and signage meet Civitas standards.
  • Maintain and update Cottage signage monthly.
  • Ensure residents receive services per their service plan daily.
  • Monitor employee morale and maintain a high level of team cohesion and unit culture.
  • Conduct daily Cottage walk-throughs for cleanliness, odors, clutter, and tour readiness.
  • Review the Resident Wellness Index (RWI) daily and coordinate appropriate interventions.
  • Review the Domo dashboard daily for falls, move-outs, behaviors, missed medications, and census.

Medication and Clinical Oversight

  • Ensure all medications are passed for each resident at each scheduled time.
  • Conduct daily wellness rounds and room sweeps, including review of narcotic sheets, med carts, med rooms, and laundry rooms.
  • Perform weekly medication pass observation with a med aide.
  • Conduct random narcotic count audits weekly.
  • Assist with medication cycle coordination, including verifying delivery, placing meds in carts, and following up on missing items.

Employee Management

  • Supervise, assign duties, and direct the work of all Cottage employees with accountability for team performance.
  • Assist with interviews for Cottage care staff as needed — does not include hiring decisions, compensation discussions, or offer authority.
  • Provide on-call supervision for the entire community on a rotating basis using the company-issued on-call phone.
  • Track staff performance, work quality, and activity attendance, and report performance concerns or attendance issues to the Wellness Director for review and appropriate action.
  • Shadow caregivers regularly to ensure proper care is being provided.
  • Assist with new hire orientation using the existing standardized training checklist.
  • Complete a shadowing shift on the floor with new hires before they work independently.
  • Follow up with new hires during training to answer questions and ensure checklist completion.
  • Ensure compliance with employment laws and company policies.
  • Recognize staff daily through coaching, validation, and Caught You Caring notes.

Family, Resident, and Vendor Communication

  • Follow up daily with families, physicians, and third-party vendors to coordinate proper resident care.
  • Lead The Cottage move-in process and assist with move-in assessments as needed.
  • Assist with entering move-in information into Eldermark.
  • Assist with care conferences and resident assessments as needed.
  • Assist with Physician Order Sheets signed every 90 days as needed.
  • Provide resident transportation services as applicable.

Administrative and Financial Oversight

  • Attend daily stand-up meeting.
  • Attend Cottage Huddles at 10am and 2pm daily.
  • Review emails and on-call phone daily for staffing needs and concerns.
  • Review and respond to incident reports, assisting the Wellness Director with follow-up interventions and ensuring appropriate documentation and resolution.
  • Assist the Wellness Director with resident assessments, service plan updates, and monthly reviews of Resident Service Plans.
  • Review KPIs and scorecards monthly.
  • Maintain awareness of census to budget, LOC revenue, expenses, falls rate, activity participation, and staff retention metrics.
  • Complete Relias training and all required training on schedule.
  • Complete monthly in-service and Dementia Live.
  • Attend all-staff meetings and required Memory Care Supervisor meetings.

KEY JOB REQUIREMENTS

  • Minimum 1 year of experience in senior living or a memory care setting required.
  • Must be able to communicate effectively with residents, families, staff, community officials, surveyors, and the general public.
  • Must have compassion for and desire to work with the elderly.
  • Must demonstrate the ability to work responsibly as a team member and as an individual.
  • Must be honest, fair, dependable, and respectful of confidentiality and the rights and privacy of others.

  • Must be computer literate; experience with Sagely, OnShift, Eldermark, Domo, and Relias preferred.
  • Experience with financial reporting and managing budgets preferred.
  • Must pass criminal background check.
  • Must meet all health requirements.
  • Must have a valid driver's license and pass a motor vehicle check.
  • Must be able to perform the essential functions of the job with or without reasonable accommodation.
