BRIEF DESCRIPTION:
The purpose of this position is to manage and supervise the Health Promotions Program. This is accomplished through the development, implementation and evaluation of diabetes and tobacco community-based interventions including environmental and system changes. Activities focus on improving the health of the community using evidence-based strategies specific to diabetes and tobacco prevention and control. This is accomplished by supervising staff to research best practices, establish goals and objectives, and evaluate activities. Other duties include collaboration with public health system partners to develop and maintain effective community-clinic linkages, and coordination to improve access to health care delivery and community-based activities promoting healthy behaviors.
LICENSING REQUIREMENTS:
Driver’s License Required – Y Type – C
OTHER REQUIREMENTS:
Experience in community health education, community engagement, and a working knowledge of health equity desired.
Ability to work evenings, weekends and extended hours.
Travel within the city as well as to training conferences within Texas is expected.

2 L Develops and achieves goals by completing program objectives and activities, developing and maintaining activity reports, ensuring all deadlines are met, coordinating activities with both internal and external entities, and working within established standards and regulations.
3 L Supervises staff by scheduling and assigning tasks, recommending additional activities, ensuring tasks are within guidelines, providing training and education, completing evaluations, and overseeing all outreach activities.
4 L Coordinates internal and external working relationships by scheduling and attending regular meetings and community-based coalitions and activities.
5 L Works with staff to develop and facilitate health education programs such as self-management/prevention workshops and tobacco cessation classes.
6 L Coordinates/maintains AADE accreditation for the diabetes self-management program.
MINIMUM JOB REQUIREMENTS:

Vocational/ Educational Preparation Requires High school graduation or GED equivalent supplemented by specialized courses/training equivalent to completion of one year of college
