The City of Verona Senior Center is recruiting for a Social Worker to join their team in January of 2026! The Social Worker serves as an advocate for Verona seniors, ages 55 and older, connecting seniors with services that help to ensure their well-being, health, and safety, with a goal of helping seniors to remain independent as long as safely possible. They this position also serves as a resource for the seniors’ families and caregivers, and are the primary contact for community referral sources. The Social Worker provides input and suggestions for programming and operational improvements that may benefit the clients of the Verona Senior Center.

The Social Worker regularly works with individuals with limited physical and cognitive abilities, juggling competing priorities and challenging situations. A minimal amount of overnight travel is required. Physical demands include lifting up to 40 pounds.

ADDITIONAL REQUIREMENTS: A valid Wisconsin Driver’s License is required. The use of a reliable insured personal vehicle is required.

- Coordinate support services for seniors, collaborating with caregivers, home care agencies, health care providers, hospice organizations, hospital discharge planners, government agencies, EMS, police, and other agencies or resources.
- Enroll clients in local and government programs, such as Medicare, Home-Delivered Meals, and transportation services.
- Develops and maintain working relationships with community health, welfare, and social agencies and seek creative means to assist clients with his/her needs.
- Conduct assessments and develop, manage, review, and update care plans that consider the client’s needs, abilities, and preferences, while maintaining the client’s dignity, safety, and independence.
- Maintain confidential client records that include assessment, case notes, and documentation of services in a timely manner.
- Assist with client Power of Attorney for Health Care (POAHC), Financial Power of Attorney, and Living Wills. Assist with POAHC capability and competency assessment as needed.
- Collaborate with Hospital and Nursing Home discharge planners to ensure ease of clients’ safe transition back home.
- Maintain an ongoing social service plan of care for each client and update it based on the results of reassessments or on-going changes in status.
- Acts as a client/family advocate, liaison and/or proxy for clients when necessary. (i.e. food pantry proxy)
- Assist with outreach efforts, including giving and/or scheduling educational presentations, making home-visits to seniors, and leading outreach activities or meetings.
- Manage and maintain an equipment and supplies loan closet, including organization, inventory, and loans.
- Develop new programs and services in consultation and collaboration with co-workers and the Senior Center Director.
- Communicate regularly with the Senior Center Director regarding current concerns and developing issues.
- Respond to public inquiries promptly and in a welcoming, patient, and friendly manner and determine need for follow-up and/or ongoing services and referrals.
- Provide general support for the operation of the Verona Senior Center as needed including opening and closing the center, greeting patrons, answering phones, and assisting with Senior Center activities, front desk, and meal deliveries.
- Submit a monthly article for the Senior Center’s monthly newsletter and assist in other areas of the newsletter as needed.
- Serve on community and county level committees as appropriate to maintain good communication with other areas and organizations offering similar services and/or resources for older adults as time allows.
- Participate in ongoing training opportunities as time and budget allow.
KNOWLEDGE, SKILLS AND ABILITIES:
- Knowledge of and adherence to a high standard of professional ethics and boundaries.
- Excellent oral, written, and interpersonal communication skills.
- Strong organizational, time management and proactive case management skills.
- Knowledge of effective assessment tools, and interviewing techniques used when working with older adults.
- Knowledge of the aging process and its effects on older adults.
- Knowledge of local and government assistance programs, including financial eligibility and enrollment procedures and benefits.
- Ability to establish and maintain positive and effective working relationships with older adults, their families, co-workers, supervisors, volunteers, referral sources, and service providers.
- Ability to conduct an effective assessment of the client, considering all factors that impact the situation, i.e. physical/mental health, family relations, cognitive issues, finances, etc.
- Ability to multi-task and manage competing priorities.
- Ability to work independently with a minimum of supervision.
- Ability to use standard computer software, including Microsoft Office, (i.e. Outlook, Word, Excel, Publisher, PowerPoint), and social media.
EDUCATION AND EXPERIENCE:
Required Education and Experience:
- A Bachelor’s degree in Social Work, Human Services, Psychology or related field from an accredited university.
- Social Work License in the state of Wisconsin.
- A minimum of two years of social work experience.
Preferred Education and Experience:
- Knowledge of Dane County and other Social Services providers preferred.
- A minimum of two years of experience working with older adults is preferred.
