Job Summary
Under general direction, is responsible for assisting with the simpler aspects of cases. Working conditions are primarily inside an office environment but may be required to work outside in all types of weather conditions for extended periods of time and occasional exposure to hazardous materials, bodily fluids, infectious diseases and unfavorable fumes, vapors and odors. Exercises no supervision.
The San Antonio Metro Health District (SAMHD) provides public health services to clients, which may include risk for exposure to and possible transmission of vaccine-preventable diseases. Staff in high-risk positions will be required to obtain relevant immunizations, perSAMHD Policy DM 13.2. Review and consideration will be given to requests for exemption from this policy.

Essential Job Functions
- Makes and takes referrals from different agencies and the general public.
- Makes presentations to community groups explaining assistance program and may participate in health fairs, meetings and community board activities; performs patient toxicity assessments; and provides Directly Observed Therapy (DOT) to patients in the clinic or in the field.
- Coordinates and conducts special events.
- Actively participates in case reviews and provides feedback to clinic supervisor and nurse case managers on requested clients; enters field visits into clinic surveillance database and completes and submits reports.
- Answers phones and provides information about programs and performs brief intakes to include counseling TB patients on the importance of taking their anti-tuberculosis medication.
- Performs related duties and fulfills responsibilities as required.
- High School Diploma or GED equivalent (recognized by the Texas Education Agency or a regional accrediting agency).
- Three (3) years of experience with a social service agency or promoting a health program.
- Valid Class “C” Texas Driver’s License.
- Any applicant selected for this position must successfully complete the City of San Antonio’s Cash-Handling Certification Training.
- Bilingual in English and Spanish.

- Unless otherwise stated, applicants are permitted to substitute two years of related full-time experience for one year of higher education or one year of related higher education for two years of experience in order to meet the minimum requirements of the job.
