Position Overview
This role ensures the effective implementation of CalAIM Community Supports and Enhanced Care Management (ECM) programs across multiple counties. The CalAIM Program Manager provides training and implementation support to staff, including Team Supervisors, Community Support Navigators and ECM Case Managers while ensuring compliance with state and county regulations.
This position collaborates with healthcare providers, multiple managed care plans, and fosters partnerships with community organizations to increase program sustainability and improve health outcomes for CalAIM clients. The CalAIM Program Manager monitors and pursues all MCP contracts and deliverables.
Expected Hours and Place of Work: The incumbent works full-time in-person in Santa Barbara. City Net office hours are Monday-Friday 8:30 am-5:00 pm, and some evenings and weekends as necessary for optimal business function.
This position requires frequent local travel, primarily in Santa Barbara County. Occasional travel throughout California may also be required as applicable.
Key Responsibilities:
Team Leadership and Management
- Hiring, training and supervising Community Supports Navigators and/or Case Managers.
- Set clear goals, expectations, and performance standards
- Address underperformance and implement improvement plans as needed
- Foster a collaborative and supportive work environment
- Knowledge, understanding and implementation of CalAIM Housing Services and Enhanced Care Management policies & procedures
Program Implementation & Performance Management
- Support development and implementation of CalAIM Community Supports quality improvement efforts and Enhanced Care Management launch through strategic, timely, and actionable workplans.
- Monitor program performance, track key performance indicators, and implement continuous improvement strategies to enhance service delivery and contract maximization.
- Participate in budget preparation and administration for Community Supports and ECM efforts, preparing cost estimates/projections, submitting justifications for requested resources, and monitoring and controlling expenditures.
Staff Support & Program Compliance
- Supervise City net staff to streamline and support client referral, enrollment and renewal processes, timely documentation, and reporting
- Develop and customize workflows and program manuals to support implementation and ensure compliance with program requirements, DHCS guidance, and managed care plans.
- Collaborate cross-departmentally with City Net leadership to develop processes to support and lead training courses on best practices for high-quality service delivery and documentation practice
- Conduct regular caseload audits to ensure appropriate and ethical documentation is maintained for all cases.
Strategic Partnership & Stakeholder Engagement
- Support the development of relationships with healthcare providers, managed care organizations, social service agencies, and other stakeholders to support client needs and program success.
- Create and support cross sector partnerships to maintain program sustainability through consistent closed loop referrals.
Qualifications
- Bachelor’s degree or higher
- 2 years supervisory experience in social services.
- Experience and proven success cultivating and maintaining productive, professional relationships with various stakeholders, partner agencies and co-workers.

- Strong interpersonal, analytical, and time management skills.
- Flexible and adaptable in response to changing client and health care providers’ needs.
- Experience working with underserved and marginalized populations
- Constantly moves about inside the office and outside to access encampments, shelters, or at meetings/events.
- Must be able to move oneself quickly in certain urgent/emergent situations.
- Must be able to lift, carry, and move up to 25 lbs.
- Will be required to drive a company vehicle or personal vehicle in the event a company vehicle is not available. Must possess a valid driver’s license and must pass a Motor Vehicle Report.
