What are the responsibilities and job description for the Behavioral Health Case Manager position at CITRUS HEALTH NETWORK?
PRIMARY RESPONSIBILITIES):
1. Facilitate the client’s understanding of the rationale, purpose and procedures associated with case management services.
2. Obtain relevant information from the client and meaningful collateral sources in order to assess the client’s current situation and identify any immediate needs, including but not limited to the safety and well-being of the client in his or her natural environment.
3. Perform Case Management Assessment, which incorporates information about client’s strengths, needs, abilities, and preferences, according to program policies and procedures.
4. Identify problems/opportunities that would benefit from case management intervention, and conduct ongoing assessment throughout the life of the case to identify emerging needs.
5. With the client, develop a written service plan that translates assessment outcomes into functional domains and measurable objectives; prioritize needs and objectives under each domain into short-term goals; and identify appropriate case management interventions.
6. Link clients to appropriate services/providers to address needs identified in the service plan.
7. Monitor client progress, by means of contact with the client, to include monthly face-to-face contacts and home visits, concerned others, and services providers to assess the effectiveness of the service plan strategies and measure progress on achieving service plan outcomes.
8. Involve meaningful collateral sources/others in initial and on-going case management service planning, as appropriate and required.
9. Advocate for the client to promote individual/family choice and self-determination and assist the client to develop self-advocacy skills, if age appropriate.
10. Appropriately discontinue case management services and effectively transition the client to the next level of care, as applicable.
11. Obtain written consent to release information from the client and/or legal guardian according to agency protocols and administrative rules.
12. Document all case management related activities on a progress note in accord with the client’s service plan and agency policies and procedures.
13. Comply with legal, funder, and agency requirements and timelines for documentation.
14. Provide services that respect the varied identities people have based on race, ethnicity, age, sexual orientation, gender and gender identification, religion, spirituality, and mental, emotional and physical differences in abilities.
15. Seek supervision, training, and technical assistance as necessary to maintain competence and enhance professional effectiveness.
16. Collaborate and/or complete prior authorization documentation, appeals, or other documents and tasks for utilization review purposes in a timely manner, as required and instructed by Program Administrator and Utilization Management Department.
17. Coordinate and integrate behavioral health and primary care services.
18. Must comply with requirements for EHR documentation based on policies and procedures including updates received via email, trainings, staff meetings, and other written communications.
19. Perform other duties assigned by supervisor or Program Administrator.
MINIMUM REQUIREMENTS:
1. A minimum of a bachelor’s degree from an accredited university or college and have preferably a year of full-time minimum of one or equivalent experience working with the target population.
2. Proof of completion of AHCA-approved case management training within 3 months of hire.
3. A valid certification from the Florida Certification Board as a Certified Behavioral Health Case Manager (CBHCM) within one year of hire.
4. Has knowledge of available resources in the service area for the target population; and is knowledgeable of and complies with state and federal statutes, rules and policies that affect the target population.
5. Proficiency in the use of computers, email, and word processing software.
6. Maintain a valid Florida driver license.
