This is a full-time on-site role located in Bolivar, MO, for a Transitions of Care Pharmacist. The Transitions of Care Pharmacist will play a key role in ensuring seamless care transitions for patients by conducting medication reconciliation, reviewing patient profiles, and counseling patients on appropriate medication use during hospital discharge and beyond. This position involves collaborating with healthcare teams, including physicians and nurses, to optimize medication therapy, prevent medication-related issues, and promote positive patient outcomes. Additional responsibilities include participation in quality improvement initiatives, maintaining compliance with regulatory standards, and providing patient education.
- Strong expertise in Medication Reconciliation and Review, with the ability to identify and address medication-related concerns.
- Proficiency in patient counseling, clinical decision-making, and optimizing medication therapies.
- Experience in collaboration with multidisciplinary healthcare teams.
- Thorough understanding of medication safety and regulatory compliance in healthcare settings.
- Licensed pharmacist with a Pharm.D. degree or equivalent; active state pharmacist license in Missouri (or eligibility to obtain licensure).
- Strong communication and organizational skills with attention to detail.
- Experience in transitions of care or hospital pharmacy is a plus.
