SALARY RANGE: $21.00-$30.00
ALL FULL TIME POSITIONS: In addition to your pay, your salary also includes Medical, Dental and Vision Insurance. The Biweekly deduction from employee paycheck is $40.00 for Employee Only, $95.00 Employee plus Children, $120.00 Employee plus Spouse, and $140.00 Employee plus family.
JOB SUMMARY: Performs nursing care procedures as a licensed practical nurse for the CHR (Community Health Representative) Program; including Case Management for clients as needed. To assist the physician and other health care providers, in the home and community settings, through patient care, case management, supportive services, and making referrals to appropriate agencies.
CPN Health Services personnel may be assigned to provide services to patients of all ages including but not limited to Indian Children and will require a criminal history background screening.
ESSENTIAL FUNCTIONS:
• Observe patients’ signs and symptoms for reaction to drugs and treatments and reports to CHR coordinator.
• Administer intramuscular, subcutaneous, and intradermal injections as ordered by a physician. Runs an annual flu shot clinic for CPN elders in the Rossville and Topeka areas, coordinated with the CHR Coordinator and CPNHS Public Health Coordinator.
• Accurately records nursing needs of patients as directed by CHR Coordinator.
• Assess client environment for safety issues and other needs and reports them as needed. Supplies clients with available resource information for needs and assists them in contacting resources if needed.
• Transporting clients as needed for errands or medical appointments.
• Fills all medication planners for clients who need assistance with their medications. Monitor compliance and keep providers informed of status. Monitors when it is time to use secondary interventions for medications if necessary.
• Performs nursing assessments as needed and contacts Providers as needed. Reports to CHR Coordinator as indicated.
• Performs patient advocacy, case management and care coordination for clients. Communicates with Providers and specialists regarding clients’ care and documents accordingly. Assists with getting Home Health, Hospice, or admission to nursing home or assisted living facilities. Works with state and/or tribal APS when needed, making visits to homes with them if required. Works with the VA system for clients and spouses to assure they receive benefits they are entitled to.
• Perform routine duties e.g., Health assessments, O2 saturation, nebulizer treatments, administration of O2, dressing changes, and other duties as necessary within the scope of their nursing practice and maintains record in the patient’s file.
• Maintains monthly report of services and submits to CHR Coordinator on the 1st business day of each month.
• Make and receive calls in a professional manner.
ADDITIONAL DUTIES:
• Perform other duties and projects as required by management.
• Make home visits ordered by physician or Senior Support coordinator.
• Other duties as required by management.
KNOWLEDGE, SKILLS, AND ABILITIES:
• Ability to communicate effectively and accurately with patients and staff.
• Ability to maintain strict confidentiality regarding all patients encounters.
• Computer skills.
• Specialized knowledge, skills, and abilities to successfully perform duties of this position.
• Certified in Basic Life Support (CPR) and keeps certification current.
MINIMUM QUALIFICATIONS FOR CONSIDERATION:
• Active license in the State of Kansas as a Licensed Practical Nurse.
• Experience in a clinic or home health setting preferred.
PHYSICAL REQUIREMENTS:
• Sitting and standing for prolonged periods of time.
• Some lifting, bending, stretching, and kneeling may be required.
ENVIRONMENTAL CONDITIONS:
• General clinical environment with frequent exposure to chemicals, bloodborne pathogens and patient body fluids.
