\n- Direct patient care: Conduct initial and follow-up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers.
- Tech → bedside integration: Train and coach staff at the point of care, standardize use of Circadia into clinical workflows.
- Data into action: Interpret trend-based insights, prioritize high-risk patients, and drive timely interventions to prevent decline.
- Close the loop: Complete documentation/charts within the same day
- High-volume rounding: Efficiently complete visits across designated SNFs (typ. 30 /day where appropriate), focusing on new admits, facility-flagged patients, and Circadia alerts.
- Quality & product feedback: Surface field insights to improve workflows and tech integration in a clinical setting
- Mentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams
- Visits: Target 30 patient visits/day
- Engagement: ≥80% Time to Acknowledgement Rate: <24h
- Documentation: Complete all notes using AI-powered tools within 24 hours of each encounter.
- Adoption & impact: Demonstrated workflow adherence at facilities; trend toward fewer falls/readmissions over time.
- Certified and licensed Nurse Practitioner in TX; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
- Eligible for Medicare participation; company bills on your behalf.
- 2 years NP experience with complex adult populations; able to practice independently.
- Valid CA driver’s license; comfortable traveling locally.
- Thrives in a dynamic, evolving startup environment.
- English required
- Tech-savvy: Google Suite, multiple EMRs, Slack, AI transcription/tools.
- Spanish is preferred.
As a full-time Nurse Practitioner, you will be employed by Circadia Health, Inc. The anticipated annual base salary range for this full-time position is $130,000 - $160,000. The base range is determined by role and level, and placement within the range will depend on a number of job-related factors, including but not limited to your skills, qualifications, experience, and location.
