What are the responsibilities and job description for the Home Modification Specialist position at CICOA?
SUMMARY
The home modification specialist will work as part of a team of professionals and para professionals to support the work of the Home Modification and Provider Relations work within the department. The specialist will work directly with the client, family, Care Manager, providers and Supervisor with the intention of creating an Exceptional Experience for CICOA clients pursuing a home modification. The specialist will manage the home modification process from start to finish with the oversight of the Home Modification/Provider Relations Supervisor.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Customer Service

Teamwork and Communication
- Demonstrates CICOA’s communication expectations when interacting with others on CICOA’s behalf
- Interacts regularly with Flourish staff to assure care coordination
- Participates in client coordination/case conferences for assigned participants
- Attends workshops, team and Departmental Meetings, General Staff meetings and trainings as directed by Assistant Director
- Accepts accountability for the management of assigned special projects at the request of Department Leadership
Care Delivery
- Conduct and provide intervention via a home visit or meeting to further assess needs under the supervision of the CM or AD.
- Provide problem resolution with home modification projects that require further oversight.
- Assist CM in care plan development utilizing person centered approach
- Communicates effectively with clients in their home or on the phone
- Remains composed and effective when faced with unexpected and uncomfortable situations
- Conducts and gathers needed assessment data and visits in client’s homes and on the telephone
- Assists with the intra-departmental transitions of clients by researching needed providers within our network
- Completes accurate, timely, comprehensive documentation
- Provide communication and answer questions from consumers, contractors, home mod providers and staff on home modification.
- Participate in the expansion of qualified home mod/ spec providers as well as other home care providers as needed
- Provide community resource referrals, participant advocacy, support, outreach and follow-up
- Demonstrates judgement and critical thinking skills in identifying client’s needs and escalating to the care team as required by departmental protocols
- Commits to an exceptional level of time management and organizational skills with the intention of streamlining the process
- Assesses the needs and eligibility for funding streams in the community to provide linkage to needed services for clients including assistance with Medicaid (MA), Energy Assistance, Food Stamps, housing legal services and other community resources
- Provide assistance with Request for Authorization service needs on the service plan
- Utilizes discretionary judgement and interventions under the supervision of the appropriate CM in order to offer strategies and comprehensive suggestions in service plan development
- Recommend improvement for streamlining policies, procedures, data collection as well as client and provider relationships
Technology
- Independently manages data entry into the electronic record as well as documents appropriate information as required
- Acquires and maintains competence in use of program hardware and software applications
- Regularly utilizes technology as part of care delivery
- Utilizes and receives referrals through the KACE System or a similar software for tracking purposes
SUPERVISORY RESPONSIBILITIES
This job has no supervisory responsibilities.
QUALIFICATIONS

EDUCATION and/or EXPERIENCE
Two years relevant experience in a medical, care management or Medicaid/Medicare setting preferred. Associates Degree in Human Services, Psychology, Nursing, or related field preferred. Healthcare experience preferred, high school diploma or equivalent required. Relatable experience working with older adults highly preferred. Experience working with state agencies and/or with Medicaid approval process preferred. Computer experience required. Must have working knowledge of email expectations, Microsoft Office, and internet functions. Experience may be substituted to meet education requirement.
