What are the responsibilities and job description for the CCS Provider position at Christian Servants Home Care?
Christian Servants Home Care, LLC is a non-medical agency that serves individuals of all ages and abilities. Our CCS services include individual skills development and enhancement, employment related skills training, and parent mentoring, adult and children mentoring. Comprehensive Community Services (CCS) is a program that helps people of all ages live their best lives. It focuses on unique needs that relate to mental health and substance use.
Start Date: 1 Month from onboarding
Duties of this position include:
- Transportation to and from the client
- Regularly scheduled client assessment meetings
- Monthly meetings with a clinical practitioner and complete supervision logs
- Weekly 1-4 hour shifts with the client providing psychoeducational mentorship or skills
development
- Daily documentation logs submitted on time and precise
- Various check-in meetings with manager
- Willing to travel between counties to client to client
To be eligible for this position, applicants must:
- Hold a Bachelor Degree in the human services field or degree related to the human service field
OR
- Hold a Master Degree in the human services field or degree related to the human service field
AND
- Experience with mental health and substance abuse
- Ability to facilitate meaningful and proactive services to clients undergoing mental health and/or substance abuse recovery
- Complete 20 hours of additional online training and competency, continuing 8 hours of training annually
In addition, applicants must:
- Complete on-boarding paperwork and training related to Christian Servants Home Care
- Have a cell phone with the ability to clock in and out and track location and complete documentation after each client visit
- Possess excellent communication skills and response skills
- Be reliable and compassionate
