Job Summary
The Physical Therapist will provide comprehensive home health services to patients in their homes, focusing on developing and executing individualized care plans. The ideal candidate will collaborate closely with the interdisciplinary team—including nursing, occupational therapy, and case management—to ensure coordinated, high-quality patient outcomes.
You will have flexibility to manage your own schedule while ensuring compliance with regulatory standards and clinical excellence.
Responsibilities
- Provide skilled physical therapy services to patients in their homes, in accordance with the physician's plan of care and agency policies.
- Evaluate patient functional status, establish treatment goals, and adjust care plans as needed.
- Document all patient visits, progress, and communication in the electronic medical record (EMR) system promptly and accurately.
- Educate patients and caregivers on therapeutic exercises, fall prevention, mobility techniques, and safe use of equipment.
- Supervise and provide guidance to Physical Therapy Assistants (PTAs) in accordance with state and federal regulations.
- Communicate effectively with physicians, nurses, case managers, and other healthcare professionals to ensure continuity of care.
- Participate in case conferences, in-service training, and staff meetings as required.
- Maintain compliance with Medicare, Medicaid, and ACHC standards for home health care delivery.
Qualifications
- Current Illinois Physical Therapist License (Required)
- Bachelor's, Master's, or Doctorate in Physical Therapy (Preferred)
- Minimum of one year of home health experience (Preferred)
- Strong background in orthopedic and post-surgical rehabilitation
- Valid CPR Certification
- Reliable, insured vehicle and current Illinois driver's license
- Proficiency with electronic medical records (EMR) and technology tools (smartphone, laptop, email)
- Excellent communication, clinical judgment, and time management skills
Compensation
- Pay: $70 – $75 per visit / hour (depending on experience and visit type)
- Flexible Schedule: Create your own patient route and availability
- Employment Options: Full-time, Part-time, or PRN
