As a Physical Therapist, You Will:
- Perform and document initial evaluations to identify patient needs and establish treatment goals.
- Develop and implement individualized treatment plans to maintain, improve, or restore physical function.
- Evaluate patient progress and modify treatment plans to maximize outcomes.
- Educate patients and families on therapeutic techniques and care plans.
- Apply infection control and universal precautions while delivering patient care.
- Ensure continuity and quality of care through timely and accurate documentation.
- Collaborate effectively with physicians, clinicians, and interdisciplinary care teams.
- Promote agency values, patient-centered care, and company policies.
- Maintain documentation in accordance with company standards and regulatory requirements.
Qualifications:
- Graduate of an accredited Physical Therapy program
- Current and active Physical Therapist license
- Minimum 1 year of Home Health experience preferred
- Current CPR certification
- Valid driver’s license and reliable transportation
- Current auto insurance
- HomeCare HomeBase experience preferred
Physical Requirements:
- Ability to stand, walk, sit, climb, crawl, squat, and kneel for extended periods
- Ability to lift, carry, push, and/or pull up to 100 lbs.
