What You’ll Do
As a Physical Therapist, you will:
- Perform comprehensive patient evaluations and develop individualized treatment plans
- Assess functional limitations and establish appropriate therapy goals
- Deliver skilled therapeutic interventions to improve mobility, strength, balance, and overall function
- Monitor patient progress and modify treatment plans as needed to maximize outcomes
- Educate patients and caregivers on exercises, safety, and ongoing care strategies
- Maintain timely, accurate, and compliant clinical documentation
- Collaborate with physicians, nurses, case managers, and interdisciplinary care teams
- Follow infection control and universal precaution standards during patient care
- Support the agency’s mission of providing compassionate, high-quality home health services
Qualifications
- Graduate of an accredited Physical Therapy program
- Current and unrestricted Physical Therapist license
- CPR Certification required
- Minimum of 1 year of home health experience preferred
- HomeCare HomeBase experience preferred
- Current Driver’s License and reliable transportation
- Valid Auto Insurance
Physical Requirements
- Ability to stand, walk, bend, squat, kneel, and climb for extended periods
- Ability to lift, push, pull, or carry up to 100 lbs.
