What You’ll Do
- Evaluate patients and develop individualized occupational therapy care plans
- Help patients improve independence with daily living activities and mobility
- Assess strength, coordination, safety, adaptive equipment, and home environment needs
- Provide therapeutic interventions that improve function and quality of life
- Collaborate with interdisciplinary teams to coordinate exceptional patient care
- Complete documentation and clinical records accurately and timely
- Educate patients and families to support long-term success at home
Qualifications
- Active Occupational Therapy License
- 1 year of Occupational Therapy experience preferred
- CPR Certification
- Valid Driver’s License & Auto Insurance
- HomeCare HomeBase experience preferred
Physical Requirements
- Ability to stand, walk, kneel, squat, and move frequently throughout the day
- Ability to lift, carry, push, or pull up to 100 lbs.
