What You'll Do
As the Clinical Manager, you'll provide leadership and oversight for all clinical operations while ensuring outstanding patient care and regulatory compliance.
Responsibilities include:
- Lead, coach, and mentor Home Health clinicians.
- Ensure compliance with Medicare Conditions of Participation (CoPs), state, and federal regulations.
- Oversee patient care, clinical documentation, and quality outcomes.
- Drive Quality Assurance and Performance Improvement (QAPI) initiatives.
- Coordinate interdisciplinary care to ensure exceptional patient outcomes.
- Support clinicians with assessments, care planning, and clinical decision-making.
- Maintain high standards of regulatory compliance and documentation accuracy.
- Promote infection prevention, patient safety, and clinical best practices.
- Foster a culture of accountability, teamwork, and compassionate care.
Qualifications
Required
- Current Registered Nurse (RN) License
- Minimum 2 years of Home Health Management experience
- Strong knowledge of Medicare Home Health regulations and Conditions of Participation
- Current CPR Certification
- Valid Driver's License
- Valid Auto Insurance
- Excellent leadership, communication, and organizational skills
Preferred
- HomeCare HomeBase (HCHB) experience
- QAPI and Performance Improvement experience
- Experience leading interdisciplinary clinical teams
