𩺠What Youâll Do
- Provide compassionate, end-of-life care to patients in their homes or care facilities
- Perform comprehensive assessments and develop individualized care plans
- Support patients and families with education, comfort measures, and emotional guidance
- Collaborate with an interdisciplinary team (physicians, social workers, chaplains)
- Monitor changes in condition and adjust care to ensure comfort and dignity
- Supervise and guide LVN/LPNs and Hospice Aides
- Maintain accurate and timely clinical documentation
- Participate in on-call rotation (if applicable)

- Active RN license (Colorado)
- Hospice or Home Health experience preferred (but not requiredâweâll train the right heart â¤ď¸)
- Strong clinical judgment and communication skills

Requirements
Current RN License
CPR Certification
Valid Driverâs License
Valid Auto Insurance
At least 2 years of Hospice RN experience preferred, not required
HomeCare HomeBase experience preferred
