In this role, you’ll provide patient-centered, holistic care to individuals facing life-limiting illnesses—supporting not just their medical needs, but their emotional and spiritual well-being. You’ll work independently in the field while collaborating closely with an interdisciplinary care team to ensure comfort, dignity, and quality of life.
What You’ll Do
- Perform comprehensive initial and ongoing patient assessments, addressing physical, functional, psychosocial, and environmental needs
- Develop, implement, and adjust individualized care plans to ensure continuity and quality of care
- Determine hospice eligibility and level of care through admission and recertification criteria
- Educate patients and families on disease progression, symptom management, and end-of-life care
- Coordinate care with physicians, social workers, chaplains, and the interdisciplinary team
- Provide clinical direction and oversight to Hospice Aides and LPNs/LVNs
- Document care thoroughly to reflect patient progress and outcomes
- Respond to changing patient needs and communicate updates to the care team
What You Bring
- Active RN license
- CPR certification
- Valid driver’s license and auto insurance
- Minimum 2 years of RN experience (hospice experience preferred)
- Experience with HomeCare HomeBase is a plus
- Strong clinical judgment, communication skills, and the ability to work independently
