The Social Worker LMSW PRN at the Tyler Hospice Branch plays a critical role in providing compassionate and comprehensive psychosocial support to patients and their families facing life-limiting illnesses. This position focuses on assessing patient and family needs, facilitating access to community resources, and delivering counseling to enhance emotional well-being during end-of-life care. The Social Worker collaborates closely with interdisciplinary hospice teams to develop individualized care plans that respect patient dignity and cultural values. This role requires flexibility and responsiveness to provide support on an as-needed basis, ensuring timely intervention during critical moments. Ultimately, the Social Worker contributes to improving quality of life and easing the emotional burden for patients and their loved ones throughout the hospice journey.
As a Social Worker, you will:
- Assesses and completes the Comprehensive Assessment within 5 days of the patient’s election of hospice care to identify patient and family’s psycho social, financial, environmental and community needs as evidenced by POC documentation, clinical records, IDG meetings and community resource referrals.
- Assists with advance directives, funeral planning, discharge planning, family counseling and may assist in changes in the level of care as indicated by the patient or family’s needs.
- Use effective interpersonal relations and communication skills to achieve desirable outcomes.
- Provides patient and family/caregiver counseling around issues of end-of-life, death, and grief.
- Attends interdisciplinary team meetings, participates in the patient care planning process and coordinating and collaborating with the IDG to promote coordination of patient care.
Provides an environment which promotes respect for patients and their privacy and property.
- Masters degree in Social Work from an accredited school
- Licensed Master Social Worker
- One or more years of Hospice experience required
- Valid Driver’s License
- Valid Auto Insurance
- AHA CPR Certification
