The Hopestone Hospice Nurse Practitioner PRN plays a critical role in delivering compassionate, patient-centered care to individuals facing life-limiting illnesses. This position focuses on managing and alleviating symptoms, coordinating care plans, and supporting patients and their families through the hospice journey. The Nurse Practitioner will work collaboratively with interdisciplinary teams to ensure holistic care that addresses physical, emotional, and spiritual needs. This role requires flexibility and responsiveness to patient needs on an as-needed basis, providing expert clinical assessments and interventions. Ultimately, the Nurse Practitioner contributes to enhancing the quality of life for patients in their final stages by delivering expert, empathetic hospice care.
As a Hospice Nurse Practitioner, you will:
- Collaborates with the IDG in assuring appropriate evaluation and palliative treatment of terminal disease.
- Consults with and educates the patient/family regarding disease process, self-care techniques, end-of-life care. Provides training to other staff as needed.
- Perform Face-to-Face visits at the patient's place of residence.
- Interacts with attending and consulting physicians as necessary regarding palliative care, in collaboration with hospice physicians.
- Provides clinical assessment results to hospice physician after making face-to-face encounters with patients, as assigned.
- Keeps up with and maintains education/knowledge in pharmacology and palliative medicine.
Knowledgeable of hospice criteria.
- Current STATE RN and Advanced Practice Nurse License
- Valid Driver’s License
- Valid Auto Insurance
- AHA CPR Certification
