The Hospice Chaplain PRN plays a vital role in providing compassionate spiritual care and emotional support to patients and their families during end-of-life journeys. This position focuses on addressing the diverse spiritual and religious needs of individuals in a hospice setting, ensuring holistic care that honors personal beliefs and values. The chaplain collaborates closely with interdisciplinary teams to integrate spiritual care into the overall treatment plan, enhancing patient comfort and dignity. By offering counseling, ritual support, and crisis intervention, the chaplain helps patients and families find meaning, peace, and hope amidst challenging circumstances. This role requires flexibility and responsiveness to meet the needs of patients on a PRN (as-needed) basis at the Overland Park Hospice Branch in Kansas.
As a Hopestone Hospice Chaplain, you will:
Assess hospice patients and support systems to identify spiritual needs as evidenced by POC, documentation, clinical records, and IDG conference minutes.
Provide support for the spiritual needs of the patient/family.
Attend interdisciplinary team meetings and participate in the patient care planning process.
Use effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes.
Meet mandatory continuing education requirements of the agency.
Demonstrate commitment, professional growth and competency by maintaining a working knowledge of public and private eligibility standards and requirements.
Promote Agency philosophy and administrative policies to ensure quality of care.
Demonstrate understanding of coordination and continuity of care by collaborating with the IDG.
Accept clinical assignments that are consistent with education and competence to meet the needs of the patients/families.
Requirements
One year of experience as a Chaplain in health care preferred
One or more years of Hospice experience required
CPR Certification
Valid Driver’s License
Valid Auto Insurance
HomeCare HomeBase experience preferred
