Choice Health at Home is seeking an experienced and compassionate Home Health Field Registered Nurse (RN) to join our growing team. T
As a Home Health RN, you will provide high-quality, patient-centered care in the comfort of patients’ homes while helping patients achieve independence, improved outcomes, and enhanced quality of life.
What You’ll Do- Complete initial and ongoing comprehensive patient assessments including physical, functional, psychosocial, and environmental needs
- Evaluate eligibility and establish appropriate levels of home health care services
- Develop, implement, and update individualized plans of care
- Educate patients and families on disease management, self-care techniques, medications, nutrition, and safety
- Determine frequency and scope of services based on patient acuity and caregiver support
- Perform preventative and rehabilitative nursing interventions
- Maintain infection prevention and standard precaution practices
- Supervise and provide clinical guidance to Home Health Aides and LPN/LVNs
- Document patient progress and clinical outcomes accurately and timely
- Communicate changes in patient condition to physicians and interdisciplinary team members
- Ensure compliance with payer requirements and quality standards
- Current RN License
- Valid Driver’s License
- Current CPR Certification
- Valid Auto Insurance
- Minimum 2 years of Home Health RN experience strongly preferred
- HomeCare HomeBase experience preferred
Compensation & Time Off
- Competitive Paid Time Off (PTO)
- Less than 1 year: 13 days
- 1–5 years: 18 days
- 5–15 years: 21 days
- 15 years: 22 days
- 7 Paid Holidays annually
