What You’ll Own
- Your territory — build, grow, and dominate it
- Your referral network — physicians, hospitals, case managers, SNFs, and community partners
- Your results — drive consistent patient admissions and exceed monthly goals
What You’ll Do
- Develop and maintain high-value referral relationships that consistently generate business
- Position Choices Home Health as the go-to provider through strategic, in-person outreach
- Identify new accounts, open doors, and close referral opportunities
- Collaborate with clinical teams to ensure smooth patient transitions and exceptional service delivery
- Stay ahead of market trends and competitor activity
- Execute targeted outreach and marketing strategies that drive admissions
- Consistently meet or exceed sales/admission goals
Requirements
- Minimum of 2 years home health or hospice sales experience preferred
- CRM Software experience preferred
- Valid Driver’s License
- Valid Auto Insurance
HomeCare HomeBase experience preferred
