LPN

Key Responsibilities

• Administer prescribed medications and treatments accurately and in a timely manner
• Perform routine nursing procedures including wound care, tube care, and injections
• Monitor residents’ conditions and promptly report changes to the RN or physician
• Document all care, treatments, and observations in the electronic medical record (EMR)
• Respond to call lights, resident needs, and emergency situations as appropriate
• Collaborate with CNAs and other team members to ensure continuity of care
• Participate in shift report and communicate important updates on resident status
• Assist with admissions, discharges, and transfers
• Maintain cleanliness, organization, and security of the medication cart
• Follow infection control protocols and all facility policies and procedures

Qualifications

• Current, active LPN license in good standing
• Previous experience in long-term care or skilled nursing preferred
• Strong clinical, documentation, and communication skills
• Ability to prioritize tasks and manage time effectively
• Commitment to providing compassionate, resident-centered care
