Job Purpose
The purpose of this role is to deliver comprehensive in-home and in-center assessments and determine appropriate levels of care for participants, ensuring their needs are accurately identified and addressed. The role involves active participation in quality assurance programs and audits to maintain and enhance service standards. As a key member of the Interdisciplinary Team (IDT), the role is crucial in collaborating with other disciplines to develop and refine individualized care plans, ensuring a coordinated and effective approach to participant care.
Duties and Responsibilities
- Receives and communicates appropriate information to staff and keeps Clinic Manager informed of changes, needs, complaints, etc.
- Conduct clinic nursing assessments to determine the nursing care needs, preferences and goals of the participants and actively participate in IDT meetings to develop participant care plans.
- Develop a plan of care based on the nursing process, which incorporates the plans of other disciplines and continuing care needs.
- Schedules and conducts home visit, to develop level of care assessments to appropriately determine participant eligibility.
- Participates in the Quality Assurance program by conducting audits and submitting results in writing.
- Performs functions related to clinical assessment, care planning, evaluation of participant care.
- Documents nursing care in the medical record.
- Able to triage participants in clinic effectively.
- On call responsibilities.
- Administers medications and maintains medication system including over the counter, contingency and emergency medication supply participants under clinic standards of practice and within scope of licensure.
- Performs phlebotomy procedures as needed.
- Participates and functions as a member of the interdisciplinary team and rotates as needed.
- Independently delivers nursing care and services to program members within scope of licensure and per primary care orders or nursing care plan.
- Mentors and assists in the orientation of new clinic staff hires.
- Performs related duties as assigned.
Qualifications
Education:
- Graduate of an accredited school of nursing; Bachelor of Science preferred.
- Licensed as a Registered Nurse in the state of California.
- BLS/First Aide certification required.
Experience:
- A minimum total of two years of personal, clinical/internship, and or employment experience providing care or services for adults with disabilities or frail elderly population required.
Skills and Knowledge:
- Effective written and oral communication skills.
- Demonstrated ability to problem solve and apply good judgment.
- Ability to function well independently.
- Ability to use or be trained in computer skills for the workplace.
- Demonstrates necessary skills and knowledge as outlined in attached position-specific Competency Assessment Profile.
Other:
- Requires physical strength to perform the job.
- Routine travel between PACE sites and in the community required.
- Requires valid driver’s license.
Physical Demands
- Must be able to remain in a stationary position 50% of the time.
- Ability to occasionally move about inside the office to access file cabinets, office machinery, etc.
- Able to operate a computer and other office productivity machinery, such as a calculator, copy machine, and computer printer.
- Able to constantly position yourself to maintain files in file cabinets such as reaching with hands and arms, kneeling, crouching, etc.
- The ability to communicate, detect, converse with, discern, convey, express oneself, and exchange information is crucial for this role.
