Position Summary: The preventive service case worker supports families from diverse backgrounds, addressing needs identified in the treatment plan to help keep families together. They assess risk and safety, work to strengthen and stabilize family functioning, and promote healthy parent-child relationships. Guided by the Program Supervisor, the case worker develops therapeutic relationships and serves as the primary service provider, aligning with the agency's family-centered approach. Looking for candidates to support the needs of children and families in Sullivan County.Responsibilities:
- Role model high performance standards daily and adhere to Agency, State, Federal, and Accreditation guidelines, striving to exceed them when possible.
- Provide parenting skills groups, advocacy, and counseling for families, addressing needs identified in treatment plans using family-centered practice approaches.
- Monitor, evaluate, and continuously enhance clinical programming while ensuring effective service delivery and collaboration with diverse clients, families, and community contacts.
- Maintain comprehensive and timely documentation, including assessments (e.g., FASP), progress notes, and case-specific materials, ensuring accuracy in client records.
- Facilitate and participate in regular planning conferences, case planning, and service implementation, ensuring the involvement of all relevant service providers.
- Collaborate and advocate on behalf of clients with DSS, community agencies, schools, and other providers, maintaining strong relationships to support program outcomes and the best interests of children and families.
- Manage a caseload of 1:16, unless otherwise directed, and meet all job requirements, including having strong communication, problem-solving skills, a valid driver's license, and cultural sensitivity.
Requirements
Education- Bachelors Degree in human services or related field required
- Bilingual - Spanish - preferred
- Some experience working with children or families in human services or related field preferred.
- Driver's License and ability to maintain insurability throughout employment required.
