Position Summary

The Prevention Facilitator empowers youth to make positive choices and achieve academic, personal, and life goals through engaging in prevention services in school and community settings. Supporting a caseload of approximately 20 students, this role delivers prevention programming, college and career readiness groups, and social-emotional learning activities while providing individualized mentoring, case management, and crisis support. The Prevention Facilitator partners with schools, families, and community organizations to foster student engagement, ensure coordinated services, and create a safe, inclusive, and supportive environment that promotes long-term youth success. The incumbent will understand and promote the organization's Mission, Vision, and Core Values to ensure alignment with the functions of the position.

Children's Aid Mission

To help children in poverty to succeed and thrive. We do this by providing comprehensive support to children and their families in targeted high-needs New York City neighborhoods. Children's Aid annually serves children and their families at over 40 sites in New York, as well as through three national program models. Children's Aid is concentrating its efforts on creating cradle-through-college pathways in high-need neighborhoods, with a focus on children's health, family stability, education, and social and emotional development.

Essential Duties and Responsibilities

  • Provide direct support to students in centers, schools and the community.
  • Manages a caseload of 20 students.
  • Facilitate Evidence-Based Programs (EBP).
  • Facilitate all assigned groups.
  • Provide individual, counseling and interventions.
  • Provide individual mentorship and referrals.
  • Assist in recruitment and retention of youth and families for prevention services.
  • Provide support and provide services to at-risk youth.
  • Accompany youth to internships, trips and other activities.
  • Provide support for all specific site activities.
  • Maintain accurate and complete records for programs and projects.
  • Assist in administering pre- and post-test evaluations.
  • Ensure timely record keeping ETO, WITNYS and other online platforms.
  • Assist in the development of local health fairs, community events and environmental strategies.
  • Adhere to the policies and procedures of Children's Aid.
  • Perform other tasks as assigned.

Minimum Qualifications

  • High school diploma required, associate degree preferred.
  • Must be credentialed after 3 years of hire.
  • 1-2 years of experience working in prevention programs or in youth development.

Key Competencies

  • Proficient in Microsoft 365 Office.

The incumbent is expected to work a schedule, whether in-person or hybrid, as determined by the department's needs to facilitate effective collaboration with the team. The position's requirements are subject to periodic review and adjustment based on organizational needs and changes in work dynamics.
