Job Summary for Children’s High Utilizer Care Coordinator:
To assist individuals in gaining access to needed medical, social, educational and other services. The primary goal is to optimize the functioning of recipients who have complex needs by coordinating the provision of quality treatment and support services in the most efficient and effective manner. Care Coordination serves to assist individuals who are not effectively connected with the services and supports they need to transition successfully from higher levels of care to effective community-based care.
- Utilizes a short-term care coordination model as outlined by LSF policy to provide resources, brokering, education referrals and other support as needed.
- Follows up and links the client with specified services and resources as identified by service providers, care givers or determined beneficial by the Care Coordinator
Requirements
Education and Experience Requirements for Children’s High Utilizer Care Coordinator:
- Bachelor’s degree in a social services field is required.
- A minimum of one year of experience working with adults in the behavioral health field.
- Good computer skills and ability to adapt to electronic health record changes.
