What are the responsibilities and job description for the Intake Coordinator position at Child & Family Support Services?
Child & Family Support Services (CFSS) is accepting applications for Full-time Intake Coordinator in Pima County.
The Intake Coordinator will conduct front door engagements and partner with children and
families to create assessments and support plans for behavioral health services. Establishing
relationships and partnering with children and families is integral to this position. We specialize
in partnering with families to build effective relationships and to inspire hope for a positive
future.
Job Responsibilities:
Conducting intakes and screenings and ensuring all clinical forms are completed and signed.
Coordinating the process of evaluations and assessments to determine initial and
on-going eligibility
Measuring and tracking progress and outcomes for young people and families.
Attending and participating in weekly Team Meetings, agency staff meetings and
Trainings
Obtaining reports from partner agencies (case managers, referring physician, hospital,
treatment facility)
Coordinates services with community resources, medical and health providers
Assisting with crisis-management and supervision when necessary.
Communicating effectively with families, Child and Family Team members (stakeholders therapists, case managers, etc), agency management and Direct SupportSpecialist Coordinators
Performing other duties related to the position
Education/Experience Needed:
Related Master's degree and prior behavioral health experience preferred
Requirements:
Must be 21 years or older
Valid Arizona Driver's License with an acceptable 39 month driving record
Reliable transportation, including proof of Insurance
Candidates are required to submit a negative drug test
Obtain an Arizona DPS Level One Fingerprint Clearance Card
Complete Adult, Child, and Infant CPR and First Aid training
