Summary

The Transition to Independence (TIP) Facilitator provides one-to-one coaching and facilitation to transitional age Youth & Young Adults (YYA) in meeting their life goals. The TIP Facilitator provides support for Young Adults using mentoring, teaching skills, counseling, and problem solving using the TIP model to maximize the likelihood of success for each YYA served. This job involves going into the community as well as being a presence at the center. This position is full-time, 40 hours per week.

  • Collaborate with the Program Director and program staff and persons served, to provide education in the community on young adults with mental health concerns and recovery principles  
  • The TIP Facilitator provides support for YYAs - using mentoring, teaching skills, counseling, and problem solving to maximize the likelihood of success for each YYA served 
  • The TIP Facilitator will be specially trained to work with YYAs experiencing first episode psychosis and substance use
  • Conducts strength-based needs assessments via the Strength Discovery Assessment process related to the transition domains as specified in the TIP System Development and Operations Manual
  • Provide case management services within the context of the YYA’s culture under the TIP model guidelines 
  • Support youth in achieving their life domain goals pertaining to housing, education, career, stabilization, mental and physical health, and creating natural supports 
  • Facilitate youth-driven programming to help engage participants in their social-emotional development 
  • Participate in professional development opportunities 
  • Transport young adults to events and activities and other appointments when pertinent 
  • Flexibility with schedule as needed to assist with overall functioning of the center

Qualifications:

Required Education and Experience

A Bachelor's degree in the social sciences, rehabilitation, and/or education fields (e.g. special education, psychology, social work) preferred. High School Diploma required. A minimum of 1-3 years' experience in a strengths-based model supporting youth or young adults to achieve their life, career, educational and housing goals through coaching, counseling, training, therapy, and/or guidance in community, educational, social service, juvenile justice, and/or mental health settings. Experience working with youth or young adults coping with trauma, mental health issues, substance abuse and homelessness is strongly preferred. Lived experience with trauma (homelessness, violence, racial, disrupted family), mental health, substance abuse, recovery and/or resiliency strongly preferred. Ability to complete company funded TIP Model Core Competency Training within 3 months of hire.

  • Cultural knowledge of key populations that will be served to address disparities in service delivery
  • Demonstrates positive attitudes towards cultural differences by showing respect and openness towards people whose social and cultural background is different from one's own 
  • Demonstrates skills for communication and interaction across cultures, including the ability to recognize and manage personal behaviors, moods, and impulses to create an inclusive, equitable, and welcoming climate within the agency 

Travel Required

Must have a valid driver’s license and reliable transportation that meets CFS Driver Policy. Travel is required to provide services in different settings and to attend agency meetings, events, and trainings
